An anal fistula is an abnormal tract or channel that develops between the end of the bowel and the skin near the anus. It often arises after an infection or abscess occurs in the glands around the anus. This results in pus formation that bursts, leading to the abnormal passageway between the gland and the skin called a fistula.
Symptoms of anal fistula include pain and swelling around the anus, sometimes boil burst open with pus or bloody discharge etc. They can cause great discomfort and develop into complications if left untreated. Surgical treatment is often required, especially for complex or recurring fistulas
Causes: Infections, abscesses, or complications from conditions like Crohn’s disease.
Symptoms: Pain, swelling, and discharge near the anus.
Fistula laser closure involves introducing the laser fibre into the fistula from the outside and passing it through to the opening inside the rectum. Then, the laser is activated, causing the tissue around it to shrink as the proteins denature.
Using the distal laser proximal ligation technique (DLPL), a minimally invasive sphincter-saving surgery, we manage to treat anal incontinence and recurrence in 99% of operated patients.
That the fistulous track must be laid open from its termination to its source is known to be the procedure of Fistulotomy.
This technique involves coring out of the fistula, usually by diathermy cautery; it allows better definition of fistula anatomy than fistulotomy, especially the level at which the track crosses the sphincters and the presence of secondary extensions.
This specialized technique uses medicated Ayurvedic threads called Kshar Sutra to cure fistula. The threads are tied through the tract to drain pus and facilitate gradual healing from inside out as granulation tissue forms. As the tract heals, the threads also get cut through and removed naturally in due course.
In this technique most of fistula is excised out and ksharsutra is ligated especially the level at which the track crosses the sphincters to save the complication of anal incontinence.

–> Gentle exercise allowed after 2 weeks
Most patients are able to resume their normal routines within 5-6 days of undergoing fistula treatment through these approaches. Weekly Follow-ups may be required to track progress.
